Transition Program

We work extremely hard to help our students become as independent as possible. We want them to be self-determined individuals that are productive, and positive members of their community. We want their future to be theirs, not what others tell them it must be.

This site is to inform parents, students, staff, and others about what we do on a daily basis. It is to act as a resource for others and hopefully motivate them to expand their vision of what our students CAN do.

Our #1 goal as a program is: to help students as individuals become as independent as possible. To that end, we work on:

  2. Backward Planning
  3. Self-Determination
  4. Functional Curriculum
  5. Daily Living Skills
  6. Employment Skills
  7. Social Skills

We want the students to understand their disability- not as a negative, but as a way of understanding how they learn best, what they need to help them, and what their limitations are. We work very hard on the above topics so that students are self-aware and are the ones in control of their future.

Student-Centered Planning- taking into account the students' wants and needs- using this as the driving force for not only their IEP, but their daily education.

The first step is to complete the Transition Survey- ask a staff member to assist you. When the survey is completed please print a copy and ask for help in moving it to your folder in the Team Drive.

What will I do?

Using this website, you will investigate and research important ideas and concepts that will help you build a path towards YOUR future. Some things you might investigate include:

  • Self-Determination and Self- Advocacy! One of the most important skills you can learn!
  • What kind of careers are you interested in?
  • What strengths and interests do you have and how do they align with a career?
  • What is your DREAM job? And what other jobs are you most interested in?
  • How to complete a job application
  • How do you create a resume and cover letter?
  • What does it mean to have an IEP?
  • What is an "accommodation" and how does it affect my life in school and in the workforce?

2. Application- This application will be a "model" for you to use in the future.
